Currently, healthcare in Victoria has financial incentives that focus on episodic service delivery instead of patient outcomes. This can lead to disconnects between service providers across the continuum of care.

Value-based care promises to deliver health services where and when they are needed most, minimising low-value services, and maximising those that deliver the best possible outcomes.
